State Society Offers Tools to Help Treat Diabetic Patients

Physicians can use the diabetes care recommendations and diabetes mellitus flow chart below when treating patients with Type 2 diabetes.

The information, based on recommendations from the American Diabetes Association (ADA), is the result of meetings held through the Pennsylvania Medical Society’s Medical Directors’ Forum, a coalition of Society physician leadership and medical directors from Pennsylvania health plans.

The recommendations review treatment goals, screening, diagnosis, and assessment and management of the diabetic patient. The flow sheet gives recommended elements for every diabetic visit and annual exam and allows physicians to record data and other notes.

You can print and make copies of the PDF below or contact Mary Ellen Corum in Practice Economics and Payer Relations at the Pennsylvania Medical Society at (800) 228-7823, ext. 2645, to receive a copy by fax or mail.
